Clearview Family
Eyecare will guarantee the accuracy of prescriptions written by our doctors
for the time period of 90 days from the date of dispensing lenses to the
patient. Within that time period, any change to prescription as deemed
necessary by our doctors will be free of charge. This guarantee is limited to 2
remakes within the specified time period. Additionally, this guarantee only
applies to lenses remade in the same design and material type as the original
lens. Any change to materials, coatings, or designs of the remade lens will
incur extra charges at the consumer’s or guardian’s expense. Our prescription
accuracy guarantee applies only to lenses and does not apply to frames, and any
change in prescription will affect only the original lenses and frame, and does
not allow for changes in frame make model or color. A change of frame as
requested by the patient will incur extra charges at the patient’s expense. The
cost of any remade lens deemed to be excluded from the terms of this guarantee
by Clearview Family Eyecare or its employees will be the sole and entire
obligation of the consumer or guardian. Furthermore, any lenses purchased at
Clearview Family Eyecare and made to specifications based on a prescription
from a doctor not associated with Clearview Family Eyecare are not covered
under the terms of this guarantee. The cost to remake such lenses will be
the sole obligation of the consumer or guardian.
1 year frame repair and adjustment warranty: We feel confident that the frames we offer are of the
highest quality, in fact, we guarantee that our frames will be well made and
trouble free. From the date of purchase,
we will warranty frames purchased from us against manufacturer’s defects for
the period of 1 year from the date of dispensing. Additionally for the period
of 1 year adjustments to the frame will be performed free of charge. If a frame is deemed to be covered under our
warranty program, the damaged components will either be repaired or replaced. The
decision of repair versus replacement lies at the sole discretion of Clearview
Family Eyecare and its employees, and not by the consumer. If a frame to be
replaced under the terms of this warranty has been discontinued by the
manufacturer, an equivalent frame will be substituted at no cost. However, if
the substituted frame differs in size and shape from the original so that the
original lenses can no longer be used, the consumer or guardian will be
financially responsible for the replacement of the lenses in the new frame. THIS
WARRANTY DOES NOT COVER LOSS OR NORMAL WEAR,
NOR DOES IT COVER DAMAGE DUE TO ABUSE OR NEGLIGENCE. If frame components are lost, or damaged due to abuse
or negligence, then the cost to repair or replace those parts are the sole
obligation of the consumer or guardian. Frame
replacement is limited to 1 replacement per warranty purchase, for the life of
the warranty. Additionally,
substitutions of the frame’s make, model, or color are not allowed under the
terms of this warranty, except under the condition of discontinuation.
1 year “vision protect” lens warranty: The purpose of
this warranty is to give you peace of mind that if your lens is defective in a
way that interferes with your lenses ability to give you clear vision, we will
replace that lens one time at no cost to you. Please keep in mind that this
warranty does not cover normal wear, such as hairline scratches. From the date of purchase, we will warranty
lenses (excluding uncoated plastic lenses) and coatings purchased from us
against manufacturer’s defects for a period of 1 year. This warranty will include
major blemishes in lenses or coating. THIS
WARRANTY EXCLUDES HAIRLINE SCRATCHES OR ANY BLEMISH WHICH DOES NOT INTERFERE
WITH THE PATIENT’S LINE OF SIGHT. THIS WARRANTY DOES NOT COVER LOSS, NOR
DOES IT COVER DAMAGE DUE TO ABUSE, NEGLIGENCE, OR IMPROPER CLEANING TECHNIQUES.
THIS WARRANTY DOES NOT COVER MINOR BLEMISHES ASSOCIATED WITH NORMAL
WEAR. Replacement of lenses under the
terms of this warranty is limited to 1 replacement of each lens, including any
originally purchased coatings, within the warranty period.
FOR PATIENTS USING VISION SERVICE PLAN:
BECAUSE VSP (VISION SERVICE
PLAN) ORDERS ARE PROCESSED AT VSP CONTRACT LABORATORIES, AND NOT BY OUR
LABORATORIES OF CHOICE, WE ARE BOUND BY THE TERMS AND CONDITIONS OF VSP
POLICIES. PATIENTS USING VSP BENEFITS TO PURCHASE LENSES ARE SUBJECT TO THE VSP
POLICIES REGARDING REPLACEMENT LENSES AND FRAMES. FURTHERMORE, OUR POLICIES REGARDING FRAME AND LENS
WARRANTIES WILL BE SUBSTITUTED BY VSP POLICIES. PLEASE CONTACT VSP FOR
